What is your financial policy?

Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money order, VISA or Mastercard.

Please note, co-pays are due prior to seeing the doctor.


What insurance do you accept?

Our practice bills all major insurance plans including: Medicare, Blue Cross, Cigna, Universal Healthcare, Bluecare, Tricare, Signature Network, Healthspring (formerly Healthnet) Network.

How do I get a refill on my prescription?

Whenever possible, prescription renewals should be called in two days before your supply is exhausted. Please have the telephone number of your pharmacy available.


What are your office hours?

Monday to Friday from 9:00 am to 5:00 pm.


What should I bring to my appointment?

Please bring your insurance card, referral if required by your insurer, test results or x-ray film, and a list of mediations you presently take.
